Below is a list of crime investigations and activities reported by the Forsyth County Sheriff’s Department in Clemmons and Lewisville last week.

• July 29 — driving while impaired/paraphernalia, 2400 block of Lewisville-Clemmons Road, Clemmons. Subject operated a vehicle while impaired. Cleared for arrest, Rodney Forrest Evans was charged with driving while impaired.

• July 29 — missing person, 6000 block of Meadowbrook Mall Court, Clemmons. Subject left nursing home and did not return.

• July 29 — police service-fraud, 6800 block of Greenbrook Drive, Clemmons. Suspect attempted to use victims Best Buy credit card.

• July 29 — violation of city/county ordinance-animal related, 6300 block of Highland Brook Drive, Clemmons. Subject allowed her dogs to bark constantly.

• July 29 — traffic accident, 3800 block of Harper Road, Clemmons. One-vehicle accident with damage of $8.000.

• July 29 — larceny, 8100 block of Riverway Road, Lewisville. Suspect took items valued at $3,000 from a trailer.

• July 30 — traffic accident, 6100 block of Stadium Drive/Ridgecrest Drive, Clemmons. Two-vehicle accident with damages of $3,500 and $1,000.

• July 30 — hit-and-run accident, 4000 block of Hampton Road, Clemmons. Two-vehicles involved with damages of $1,500 and $1,100.

• July 30 — police service-welfare check, 4000 block of Kinnamon Road, Clemmons. Unable to locate a subject on a welfare check.

• July 30 — welfare check, 7000 block of Orchard Path Drive, Clemmons. Subject stated that he wished to harm himself.

• July 30 — welfare check, 800 block of Lewisville-Vienna Road, Lewisville. Family member wanted a welfare check of four-year-olds.

• July 30 — drug violations, 200 block of Fairhaven Road, Lewisville. Marijuana found during suspicious vehicle call.

• July 31 — trouble with juvenile, 1100 block of South Peace Haven Road, Clemmons. Juvenile walked away from day camp.

• July 31 — traffic accident, 6300 block of Idols Road/Hampton Road, Clemmons. One-vehicle accident with damage of $5,000.

• July 31 — traffic accident, 2400 block of Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $3,000 and $1,800.

• July 31 — police service-information provided, 100 block of Stadium Oaks Drive, Clemmons. Caller received unknown toll bill in the mail.

• July 31 — hit-and-run accident, 3100  block of Gammon Lane, Clemmons. Two-vehicles involved with damages of $1,500 and $800.

• July 31 — police service-non-criminal call for service, 1800 block of Ammons Drive, Clemmons. Items taken without permission from a rental home.

• July 31 — indecent exposure, 6200 block of Ramada Drive, Clemmons. Intoxicated male was causing problems at a hotel.

• July 31 — traffic accident, 2300 block of Lewisville-Clemmons Road, Clemmons. One-vehicle accident with damage of $1,200 to a gas station awning.

• July 31 — driving while impaired, 1400 block of Lewisville-Clemmons Road, Clemmons. Subject was driving under the influence. Cleared for arrest, Jessica Marie McCann was charged with driving while impaired.

• July 31 — disturbance/non-criminal, 6500 block of Shallowford Road, Lewisville. Argument regarding cyclists making it difficult to pass.

• July 31 — breaking and entering without force, 1000 block of Chockecherry Lane, Lewisville. Subject trespassed on private property.

• Aug. 1 — harassment (non-criminal), 3000 block of Forest Line Drive, Clemmons. Subject was harassed through email.

• Aug. 1 — traffic accident, 6800 block of Idols Road, Clemmons. Two-vehicle accident with damages of $1,800 and $800.

• Aug. 1 — larceny, 8600 block of Shallowford Road, Lewisville. Offender suspected of tampering with power equipment.

• Aug. 1 — simple assault-non aggravated assault, 1300 block of Lewisville-Clemmons Road, Lewisville.  Subject caused a disturbance at a bar and assaulted subjects. Cleared for arrest, Crystal Michelle Newsome was charged with simple assault.

• Aug. 1 — threatening suicide, 200 block of Brookfarm Trail, Lewisville. Subject threatened suicide.

• Aug. 2 — police service-non criminal call for service, 3600 block of Clemmons Road, Clemmons.  Employee made jokes about shooting his boss.

• Aug. 2 — hit-and-run accident — 2600 block of Lewisville-Clemmons Road, Clemmons. Damage of $1,800 to vehicle.

• Aug. 2 — breaking and entering without force, 4100 block of Hampton Road, Clemmons. Subject entered residence by unlocked door to look around.

• Aug. 2 — police service-welfare check, 3100 block of Gammon Lane, Clemmons. Caller wanted to check on his daughter.

• Aug. 2 — fleet vehicle accident, 2700 block of Lewisville-Clemmons Road, Clemmmons. Vehicle collided with vehicle in front of it. Damages of $1,700 and $200 to vehicles involved.

• Aug. 2 — larceny from building, 400 block of Williams Road, Lewisville. Phone was stolen in a bathroom. Phone was valued at $800.

• Aug. 3 — fraud, 6300 block of Clemmons Point Drive, Clemmons. Reporting person lost card.

• Aug. 3 — autobreaking and larceny, 6400 block of Sessions Court, Clemmons. Vehicle window broke and items stolen from inside with a total stolen value of $300.

• Aug. 4 — suspicious person, 6300 block of Cook Avenue, Clemmons. Reporting person thought she saw someone in her apartment.

• Aug. 4 — counterfeiting, 6800 block of Shallowford Road, Lewisville. Three unknown suspects attempted to use counterfeit money.
